This Privacy Policy (“Policy”) explains how Parsons Smile Center and Dr. Fady Salha, DMD (“Parsons Smile Center,” “we,” “us,” or “our”) collect, use, disclose, and safeguard information when you visit parsonssmilecenter.com and any associated pages or forms (collectively, the “Website”). Please read this Policy carefully. By using the Website, you acknowledge that you have read and understood it.

1. Scope & relationship to HIPAA

This Policy governs information collected through the Website. It is separate from, and does not replace, our Notice of Privacy Practices (NPP) issued under the Health Insurance Portability and Accountability Act of 1996 (“HIPAA”), which governs Protected Health Information (“PHI”) created or maintained in connection with your dental care. Please do not transmit medical history, diagnoses, or other PHI through Website forms or email. Our online forms request scheduling information only. Communications regarding your treatment should occur through the secure channels our office provides. A copy of our NPP is available at either office upon request.

2. Information we collect

Information you provide. When you complete our appointment-request form or contact us, we may collect your name, email address, telephone number, preferred office location, preferred appointment date, general treatment of interest, and any message you choose to include.

Information collected automatically. When you visit the Website, our hosting and security systems may automatically log technical data such as your IP address, browser type and version, device and operating system, referring/exit pages, and the dates and times of your visit. This data is used for security, diagnostics, and to improve the Website.

Cookies and similar technologies. The Website uses strictly necessary cookies required for core functionality and to protect against spam and abuse. Embedded content (such as Google Maps) may set its own cookies. You can control or delete cookies through your browser settings; disabling certain cookies may affect Website functionality.

3. How we use information

  • To respond to your inquiries and schedule, confirm, or reschedule appointments;
  • To operate, secure, maintain, and improve the Website;
  • To send administrative communications you request and, where permitted, appointment reminders;
  • To detect, prevent, and address fraud, abuse, security incidents, and technical issues; and
  • To comply with applicable laws and enforce our terms.

4. How information is shared

We do not sell your personal information. We may share information with:

  • Service providers who perform functions on our behalf (for example, website hosting and the form-processing tool that stores appointment requests), under contractual confidentiality and data-protection obligations;
  • Google LLC, in connection with embedded Google Maps, Google Fonts, and the display of our public Google reviews via the Google Places API, subject to Google’s Privacy Policy;
  • Legal and safety recipients, when we believe disclosure is required by law, subpoena, or governmental request, or is necessary to protect the rights, property, or safety of our patients, staff, or others; and
  • Successors, in connection with a merger, acquisition, or sale of assets, subject to this Policy.

5. Third-party services & links

The Website integrates and links to third-party services (including Google Maps, Google reviews, and review platforms such as Yelp and Healthgrades). We are not responsible for the privacy practices or content of third parties. We encourage you to review the privacy policies of any third-party service you use.

6. Data security

Consistent with the New York Stop Hacks and Improve Electronic Data Security (SHIELD) Act and applicable law, we maintain reasonable administrative, technical, and physical safeguards designed to protect information collected through the Website. However, no method of transmission over the Internet or electronic storage is completely secure, and we cannot guarantee absolute security.

7. Data retention

We retain Website inquiry and appointment-request information only for as long as necessary to fulfill the purposes described in this Policy, to comply with our legal, regulatory, accounting, or reporting obligations, and to resolve disputes, after which it is deleted or de-identified. PHI in your dental record is retained in accordance with applicable law and our records-retention policy, separate from this Website.

8. Children’s privacy

The Website is intended for a general audience and is not directed to children under 13, and we do not knowingly collect personal information from children under 13 in violation of the Children’s Online Privacy Protection Act (“COPPA”). Appointment requests for minors should be submitted by a parent or legal guardian. If you believe a child has provided us information through the Website, please contact us so we can delete it.

9. Your privacy rights & choices

Depending on your state of residence, you may have the right to request access to, correction of, or deletion of personal information we hold about you, to opt out of certain processing, and to not be discriminated against for exercising these rights.

California residents (CCPA/CPRA). If you are a California resident, you may request disclosure of the categories and specific pieces of personal information we have collected, request deletion or correction, and opt out of any “sale” or “sharing” of personal information (we do not sell or share personal information as those terms are defined by law).

To exercise any right, contact us using the details in Section 12. We will verify your request and respond within the timeframes required by applicable law.

10. Communications & “Do Not Track”

If we send appointment-related or marketing communications, you may opt out at any time by following the unsubscribe instructions or contacting our office; opting out of marketing will not affect essential communications about your care. Because there is no common industry standard for “Do Not Track” signals, the Website does not currently respond to them.

11. Changes to this Policy

We may update this Policy from time to time. The “Last updated” date above reflects the most recent revision. Material changes will be posted on this page, and your continued use of the Website after changes take effect constitutes acceptance of the revised Policy.
