Parsons Smile Center is required by law to maintain the privacy of your protected health information (PHI), to provide you with this Notice of our legal duties and privacy practices, and to follow the terms of the Notice currently in effect.

- Treatment. To provide, coordinate, or manage your dental care and related services, including with other providers involved in your care.
- Payment. To bill and collect payment for the treatment and services you receive, including verifying coverage with your insurance carrier.
- Health Care Operations. For quality assessment, staff training, scheduling, appointment reminders, and administrative activities necessary to run the practice.
- As Required by Law. When federal, state, or local law requires the use or disclosure.
Your Rights
- Inspect and obtain a copy of your health record.
- Request a correction or amendment to your health information.
- Request confidential communications (e.g., by a specific phone number or address).
- Request restrictions on certain uses and disclosures.
- Receive an accounting of certain disclosures of your health information.
- Obtain a paper copy of this Notice on request, even if you agreed to receive it electronically.
Complaints
If you believe your privacy rights have been violated, you may file a complaint with the practice or with the U.S. Department of Health and Human Services, Office for Civil Rights. You will not be retaliated against for filing a complaint.
